Models, here are all the forms you need to fill out prior to a shoot. If you're not willing/able to fill out any one of these forms, we will not consider scheduling a session with you. Please review each one carefully and contact us with any and all questions you have prior to your shoot.
*All signatures on all forms must take place with a Clipnic represtative present to witness. Any forms signed without Clipnic witness will be rejected. |

Model Release
|
Our resellers require that "If the images that you are submitting have an identifiable human face, then you will need a model release". Therefore we require all models to complete a new Clipnic Model Release for each and every session. We have taken the wording of our release directly from our reseller's sample releases, so if you have any questions about this document we will do our best to help you understand it. But we are not willing/able to make any amendments and/or changes to our release. So if you don't like, understand or agree with anything in our Clinic Model Release, please, do not sign it! IRS Tax Form
|
We are required by law to provide the IRS with an accounting of all persons we pay more than $600 per year. Therefore we require each and every paid model to fill out and sign this standard W-9 form once per tax calendar year. After you make more than $600/year from us, we will generate a 1099 (wherein we do NOT withhold any taxes) and submit it to the IRS. This means that you are soley responsible for all your taxes. Again, we do not withhold any taxes from your pay.

Clipnic Agreement
|
Before we schedule a shoot with any new model, we require a completed Clipnic Model Agreement. This agreement states all Clipnic expecations, rates of pay, and lots of other important information. We will provide this document at each face-to-face casting meeting or you can download the Clipnic Agreement here.

Talent Profile
|
To help us better understand how we can use your modeling services, we ask that all models fill out a Clipnic Talent Profile. This document simply lists some activities we're interested in shooting - and gives you space to tall us all about your talents and why we should use you in our Clipnic Stock Footage. This document is the only one NOT required, but it will help us find you more work! :)
